“A Show of Gratitude”
The Birthday Club is a show of gratitude by AA members who contribute $2.00 or more for each year of sobriety during the month they became sober.
These funds are used to help support Central Office staff and volunteers to carry the message to the alcoholic who still suffers.

You will be assigned an official @edmontonaa.org mailbox for the service position you hold. If you need mail forwarding to your private email address, be sure to mark the checkbox requesting that before submitting. Questions or Concerns can be directed to our Email System Administrator or you can leave a note at the bottom of this form.

Faithful Fivers Club Pledge Form
The “Faithful Fivers Club” was initiated several years aga to generate funds for the Twelfth Step work done through the staff and volunteers at Edmonton Central Office.
AA members pledge a donation of $5.00 or more a month!
What you receive for your support is the satisfaction of carrying the message to the alcoholic who still suffers, and recognition (if your desire) in the monthly ECO newsletter.

Corrections Volunteer Application Form
Sponsored by the Corrections Committee
The Corrections Committee is responsible for hosting AA meetings at each Edmonton Area Correctional Institution, having a liaison at each facility, and maintaining rapport with institution staff.
The Agreement
- To be eligible for AA service work in the area of Corrections, it is suggested that volunteers have completed the steps. It is further suggested they have at least 1 year of continuous sobriety in the program.
- Volunteers should be willing to make a commitment of at least 1 meeting per month at one of the institutes.
- Correction committee volunteers must complete and submit this form. They will also need an Alberta Justice / Solicitor General or CPIC clearance form, both of which require a front and back copy of a currently valid Alberta Driver’s License.
- Volunteers will be given information and guidelines pertaining to security from the applicable institution and will conduct themselves according to these guidelines and in a manner befitting AA’s Traditions and Concepts.
- If any of these conditions are not met, or they are violated, the Corrections Committee may invalidate clearance at any time.
Election Nomination Form
Intergroup Officers are elected bi-annually, typically serving a two-year rotation. Central Office Board members are also elected for 2-year terms, although their position must be reaffirmed each year by the Intergroup membership body at the Annual General Meeting (AGM) held in December. Board members may choose to allow their name to stand for one additional 2-year term.

Website Feedback Form
When AA began, the medium for providing information to the general public was the printed word: newspapers, books, magazines. Now it is the internet and social media.
The Edmonton AA website can be a powerful public information tool that can reach many people. How influential is the internet? For a lot of people, it is their main form of communication, information, and entertainment. These days more people spend time on the internet than watching television.
It’s important that we receive suggestions from the community we serve regarding the function and content of the website. Please use the following online form to send your comments and suggestions on how we might improve the site to the Edmonton Intergroup Communication Services committee.

This information is used by Edmonton Central Office staff and volunteers for quick reference in the office. Note: The personal phone numbers and email address of members are kept confidential and are never given out by staff or volunteers at ECO without prior permission.
